Background of the Study:
Free healthcare programs are designed to remove financial barriers and improve access to essential health services. In Kogi State, maternal health has been a focus of government and donor-led interventions aimed at reducing maternal mortality and morbidity. By offering free or subsidized healthcare services, these programs aim to encourage pregnant women to seek regular antenatal care, skilled birth attendance, and postnatal services (Okeke, 2023). Theoretically, free healthcare should lead to improved maternal outcomes by increasing service utilization and reducing the economic burden on families.

However, the real-world impact of free healthcare programs is influenced by multiple factors beyond cost. In Kogi State, challenges such as limited healthcare infrastructure, shortages of skilled personnel, and logistical issues in service delivery can undermine the benefits of free healthcare. Additionally, social determinants such as education, cultural beliefs, and geographic accessibility play significant roles in maternal healthcare utilization. While some studies have shown positive trends in maternal health outcomes following the introduction of free healthcare services, others have indicated that the quality of care and sustainability of these programs remain critical concerns (Ibrahim, 2024). The effectiveness of free healthcare programs in Kogi State thus requires a comprehensive evaluation to determine their impact on maternal health indicators such as antenatal care attendance, institutional deliveries, and maternal mortality rates (Fatima, 2025).

Statement of the Problem:
Despite the implementation of free healthcare programs in Kogi State, maternal health outcomes remain suboptimal. Many women still face challenges in accessing quality maternal care, as free services alone do not overcome barriers related to geographical distance, inadequate facility infrastructure, and a shortage of trained healthcare providers. In some instances, the removal of financial barriers has led to an increased patient load, which, when coupled with insufficient resources, results in compromised quality of care. This scenario has contributed to persistently high rates of maternal mortality and complications during childbirth (Umar, 2023). Moreover, cultural factors and low health literacy further impede the effective utilization of free healthcare services. These issues highlight a gap between policy intentions and on-the-ground realities, underscoring the need for an evaluation of how free healthcare programs are affecting maternal health outcomes in Kogi State.

Objectives of the Study:

  • To assess maternal healthcare utilization rates under free healthcare programs in Kogi State.
  • To evaluate the impact of free healthcare on maternal health outcomes, including antenatal care and institutional deliveries.
  • To recommend strategies for enhancing the quality and reach of free healthcare services.

Research Questions:

  • How have free healthcare programs affected maternal healthcare utilization in Kogi State?
  • What is the impact of these programs on maternal health outcomes?
  • What improvements can be made to increase the effectiveness of free healthcare for maternal services?

Research Hypotheses:

  • H1: Free healthcare programs are associated with increased utilization of maternal health services.
  • H2: Improved access to free healthcare leads to better maternal health outcomes.
  • H3: Addressing infrastructure and staffing issues will further enhance the benefits of free healthcare programs.

Scope and Limitations of the Study:
The study focuses on public maternal healthcare facilities in urban and rural areas of Kogi State. Limitations include potential variability in program implementation and challenges in measuring quality of care.
